Many Parents, One Vine
Many Parents, One Vine is a parenting support group which meets on Sunday mornings during the formation hour. MPOV is a space to grow in faith and connect with other parents in mutual affirmation. Raising children takes a village! Parents and outside speakers facilitate Sunday conversations focusing on podcasts, books, and timely topics in faith and parenting. The MPOV leadership team includes Andrew Robertson, Scott Mates, Julie Farmer, and Paula Chambers. If you are interested in learning more about this offering, please indicate your interest below.
This year we are also hoping to offer a small group for parents of young children. Parenting in the earliest years of a child’s life takes a particular kind of care, and we hope this small group will offer a place for young parents to speak candidly about the joys and challenges of raising small children in faith. Group sessions may include conversation, prayer, and time for fellowship. Please indicate if you are interested in this offering and we will be in touch with more information. Registration will be included in the small group sign-up form, opening on September 5.
Many Parents, One Vine is a parent-led support group that addresses topics relevant to family life today. This group will meet most Sundays beginning Sunday, September 15 through Sunday, May 11. All are welcome and no registration is required.
I am interested in being part of this formation offering and community of parents.
Many Parents is parent-led. The MPOV leadership team chooses the themes, topics, and resources we use for each session. They also share responsibility for facilitating group conversation each Sunday. Would you like to know more about joining the MPOV leadership team?

I parent/guardian, DO NOT provide consent for ministry leaders to communicate directly with my youth for ministry purposes. I, the parent, am the primary contact in lieu of my youth.
Does your child have any allergies or dietary restrictions (e.g. vegetarian, vegan, gluten-intolerant, etc.)? If your child needs an epi-pen, please indicate that so that a staff member may contact you.
We are striving to create ministries for children and youth that reach a range of abilities. Does your child have any developmental or sensory needs or learning differences which would help inform our inclusive planning?

4th and 5th Grade
We will cultivate a 4th and 5th grade community by offering specific opportunities for formation, outreach, and relationship-building. We are shaping the formation hour on Sunday mornings to include time for these additional opportunities in conjunction with the Level 3 Atrium. The 4th and 5th grade community offers specific opportunities for formation, outreach, and relationship-building. The Sunday morning formation hour includes these offerings in combination with Level III catechesis. Sixth graders may elect to remain with this group on Sunday mornings or to join youth Bible study group which happens concurrently.

Deepening Grace: Sacraments for Children and Parents (2nd-4th grade)
We plan to offer this special formation course especially for children in grades 2-4 and their parents from January 26 through March 2, 2025. It would be helpful to know of your family’s potential interest in this offering and we will be in touch closer to time. Previously, this formation course has been known as Solemn Communion.
Would you be interested in learning more about Deepening Grace: Sacraments for Children and Parents for yourself and the child you are registering?

Palmer Hall Ministries Registration (children and parents)
On Sundays, St. Stephen's offers the Palmer Hall service, which takes place in Palmer Hall chapel in the parish house. The Palmer Hall service, offered September through May, is designed particularly with families in mind, especially those with babies and young children. The liturgy follows the familiar form of Holy Eucharist found in the Book of Common Prayer and draws on a variety of elements from all over the world to make it more accessible to children. Children ages five through 5th grade are invited to serve as acolytes (carrying the cross, Gospel book, or torches). Children in 3rd through 5th grades are invited to serve as readers.
Please select all ministries below in which your child wishes to serve in Palmer Hall this year. Training will be offered.
Child: Acolyte
Child: Reader
Parents also join their children in serving during the Palmer Hall service. These roles include chalice bearers (serving the wine during communion), ushers (greeting and orienting attendees), altar guild (setting the altar before the service), and flower guild (preparing fresh flower decorations for the chapel each week). Please select all ministries in which you wish to serve in Palmer Hall this year.

Music
We love to incorporate your child's talents into worship where possible. If your child plays an instrument and would consider playing during a worship service (Palmer Hall, the Agape Meal, etc.) please let us know their name and instrument.
St. Stephen's Choristers, 1st - 6th grades
We are excited to offer our choir for young people of all genders and levels of musical experience for the 2024-25 program year. If your young person is interested in participating in choir, please indicate that below.
Yes, my child would like be a St. Stephen's Chorister (1st - 6th grade)
No previous musical experience is required to participate, and we invite you to tell us about your chorister's musical background (e.g. new to singing/choir, can read music, any participation in school ensembles or music lessons, singing or instrumental experience, etc).
Please indicate below if you would be interested in serving as a parent volunteer in support of the chorister program. A member of our music staff will contact you with more information.

Youth Bible Study
Youth Bible Study is offered during the formation hour between worship services most Sundays, and we do not need to know in advance exactly when your youth will attend, but it is helpful to have a sense of frequency. This bible study is an especially helpful option for those who may not be available for Sunday Evening Youth Group.
Youth Bible Study Attendance
My youth will be in attendance
Youth Group Parent Supports: youth group is a gift we give to our children, and most events require a few helpers to make things run smoothly and safely. We will distribute a sign-up calendar with all the opportunities to serve. Parents are expected to help about three times throughout the year.
YES, I will help! Share the sign-up calendar.

Liturgical Youth Ministries: Ushers (6th-8th); Acolytes (9th grade and up)
Training is required for acolytes and youth ushers, irrespective of prior experience. Visit our Youth Acolyte and Usher webpage for information about these ministries, expectations, and responsibilities.
About scheduling: unless you instruct us otherwise, we group siblings together on ministry teams and also, to the extent possible, coordinate sibling schedules between youth ushers and acolytes. We also attempt to schedule friends together, so we welcome requests in the form below, acknowledging also that we may not be able to enact them. Acolytes and ushers schedule reminders and substitutions are administered through Minsitry Scheduler Pro (MSP), which St. Stephen's uses to manage all liturgical minsitries. Note: the primary parent/guardian email(s) is included in all MSP communications; see the field below for adding additional email contacts.
Select your youth's ministry:

Youth Usher (6th-8th grade)

MINISTRY SCHEDULER PRO: This useful system manages reminders and substitutes. For acolytes, we include BOTH the youth and primary parent email addresses in communications. IF an additional parent email should be included in MSP, please list it below. Generally, youth ushers are not responsive enough to manage MSP themselves, so we ONLY include the primary parent contact. IF an additional email should be included for a youth usher, please list it here as well:
What are the names of sibling(s), family, or friend(s) you wish your registrant to serve alongside on their ministry team? We will try to honor your requests but cannot always do so.
NEW: YOUTH READERS in grades 9-12 (winter/spring 2025). We are gathering indications of interest for youth who wish to audition this fall to serve in the new year as readers in worship services. Readers would likely serve 1-2 times each season and would be eligible to read at the 9 and 11:15 a.m. services, the 5:30 p.m. service if desired, and the summer 10 a.m. service.
Yes, I'm interested. We will follow up later this fall. Thank you!
Parent/Guardian

Please share any additional information the family ministry team or youth leaders should know, or contact the Caroline Harris
Final question: Is there other important information you would like us to know about your family that was not covered elsewhere in this form? If so, please share below or feel free to directly contact a member of the family ministry team.
Thank you for registering for family ministries at St. Stephen’s. You will receive an email confirming your submission. There will be a link to this form in your confirmation for the option to register another child.
